<講演概要>
The life expectancy for men and women in Japan in 2014 is the highest ever recorded, according to a report released by the Ministry of Health, Labor and Welfare.

The average life expectancy for Japanese men is 80.5 years of age, while for women, life expectancy was calculated at 86.8 years of age. Both groups increased by 0.29 and 0.22 years respectively over 2013. (Japan Today 1 August 2015)
So what does this trend imply for the dental profession? More and more old and very old Japanese persons will require dental care, and natural teeth are being retained until later in life, although edentulism is still a reality in old age. These developments present a considerable challenge as patients often present with compromised general health as well as challenging conditions for implant placement. However, the benefits of dental implants are well documented and the survival and success rates similar to younger patient cohorts. Nevertheless, elderly patients often decline the intervention, mainly motivated by fear of surgery and pain. Therefore, applying minimal-invasive operation techniques may increase the acceptance of implant treatments. Recent evidence encourages the use of shorter and small diameter implants as a predictable treatment option with favourable survival rates, which furthermore widens the treatment spectrum for the edentulous mandible. Implant acceptance in the elderly patients may also increase when the overdenture is easy to manage and the abutment straightforward to clean. For all these reasons it seems time for a paradigm shift towards low-tech and minimal invasive treatment concepts. The possible benefits from an implant treatment in elderly adults might reverse some of the functional, psychological and psycho-social effects of tooth loss and thus increase the oral health related quality of life in old, and even very old Japanese.
